Cystic lymph node metastasis in patients with head and neck cancer: An HPV-associated phenomenon.

چکیده
BACKGROUND : Cystic lymph node metastases have been associated with tonsil cancer. A subset of oropharyngeal cancers contain human papillomavirus (HPV) DNA. The clinical and virologic associations of cystic nodal metastasis in head and neck cancer (HNSCC) were investigated. METHODS : A retrospective review of patients undergoing neck dissection between 2002 and 2004 identified patients with cystic lymph node metastases. Clinical and pathologic information was recorded. The presence of HPV DNA was analyzed using in situ hybridization in a subset of patients. RESULTS : One hundred neck dissections were reviewed and 20 had cystic nodes. Seventeen of these patients had primary tumors arising in the palatine or lingual tonsil. Three were "unknown primary." HPV DNA was present in 11 of 13 patients with cystic metastases with tissue available for analysis (87%). No HPV DNA was identified in tumor from 21 patients with solid nodal metastasis (p <.0001). CONCLUSION : Cystic cervical lymph node metastasis is strongly associated with HPV-related tonsillar HNSCC: (c) 2008 Wiley Periodicals, Inc. Head Neck 2008.

Nasopharyngeal Carcinoma with Cystic Cervical Metastasis Masquerading as Branchial Cleft Cyst: A Potential Pitfall in Diagnosis and Management
Introduction:Most metastatic lymph nodes from head and neck malignancy are solid. Cystic nodes are found in 33% - 61% of carcinomas arise from Waldeyer’s ring, of which only 1.8% - 8% originate are from the nasopharynx. Some cystic cervical metastases were initially presumed to be branchial cleft cyst. This case report aims to highlight the unusual presentation of cystic cervical metastasis sec...
متن کاملDETERMINATION OF THE PREVALENCE OF OCCULT NECK LYMPH NODE INVOLVEMENT IN PATIENTS WITH LARYNGEAL SQUAMOUS CELL CARCINOMA
Background & Aims: Squamous cell carcinoma (SCC) accounts for 85% of laryngeal cancers. In some cases, lymph nodes do not show any specific symptoms despite being involved by cancer. Previous studies showed that patients undergoing selective neck lymph node dissection (SND) had significantly lower recurrence rates. We investigated the prevalence of occult neck lymph node involvement in patients...
متن کاملCorrection: Human Papillomavirus and Cystic Node Metastasis in Oropharyngeal Cancer and Cancer of Unknown Primary Origin
The clinical significance of human papillomavirus (HPV) in neck node metastasis from cancer of unknown primary (CUP) is not well established. We aimed to address the relationship of HPV status between node metastasis and the primary tumor, and also the relevance of HPV status regarding radiographically detected cystic node metastasis in head and neck squamous cell carcinoma (HNSCC) and CUP. HPV...
متن کاملHigh-risk human papillomavirus and cervical lymph node metastasis in patients with oropharyngeal cancer.
BACKGROUND The purpose of this study was to determine the role of high-risk human papillomavirus (HPV) in lymph node metastasis and the depth of invasion in oropharyngeal cancer. METHODS The study included patients with 90 oral carcinomas and 66 oropharyngeal carcinomas. High-risk HPV in situ hybridization was performed to detect HPV infection. RESULTS The positive rate of high-risk HPV in ...

Nasopharyngeal carcinoma, particulary tumors endemic to the Far East, commonly harbors Epstein-Barr virus. The detection of nuclear antigen associated with EBV and viral DNA in NPC cells have revealed that EBV can infect epithelial cells and is associated with transformation. Human papilloma virus is an epitheliotrophic oncogenic virus that has been detected in a variety of head and neck tu...
